Apply for Vet Coop

If you would like to participate in our Veterinary Cooperative for Apocaps by using the supplement under the supervision of your local veterinarian, please do the following.

1) Download the Authorization form here:-> click here for the latest authorization letter

2) Ask your veterinarian to fill out and sign the authorization form and have them fax it to us at 808-442-1180.

3) We will ship a complimentary supply for you to pick up at your veterinarian’s office.

This is a limited time program and we may cancel it at any time.

UPDATE: As of 4/30/2009 – we have a waiting list for the veterinary cooperative program because of a shortage of funds, if you would like to donate to keep the vet co-op program running so we can provide free supplies of the supplement, please visit this link to help maintain our program
